We are writing today to inform you of some exciting changes to our medical practices beginning February 1, 2017.
Dr. Craig Birkby
After 17 years as Director of the Seattle Skin Cancer Center and working in the Nordstrom Medical Tower, I will be moving my practice to Edmonds, WA, just minutes away from my home. I have the opportunity to join two outstanding and highly respected dermatologists, Janet Trowbridge MD, PhD and Melanie Kuechle MD at Puget Sound Dermatology. My practice will remain the same, focusing exclusively on the diagnosis and treatment of skin cancer and pre-cancers, with an emphasis on Mohs surgery.
Dr. Daniel Berg
After 17 years at the University of Washington and over 3 years working with Dr Birkby at Seattle Skin Cancer Center, I will be joining the Polyclinic as their first Mohs surgeon. I will continue to focus on providing surgical services, primarily in the treatment of skin cancer with an emphasis on Mohs surgery. I will continue to see patients in the Nordstrom Tower, 1229 Madison St, Ste 1090, Seattle.

Your wellness and health are priorities to us. Most of our patients have been referred to us for surgical treatment and have returned to their referral doctor for continued skin cancer monitoring and care. A summary of your care was sent to your doctor at the time of your surgery.
For those of you who see one of us as an established patient, we encourage you to continue appropriate medical care in one of the following ways:
Option 1: Establish care with Dr. Birkby at his new practice and location in Edmonds, WA.
Option 2: Establish care with Dr. Berg at his present location and The Polyclinic.
Option 3: Establish care with another dermatologist of your choosing.
Your medical records are confidential, and a copy can be transferred to the doctor of your choice with your signed consent. If you wish to transfer your medical records please contact our office for the appropriate consent form at 206-215-3300 prior to January 15, 2017. After February 1st your records will be held at Puget Sound Dermatology. Scanned medical records will be provided electronically at no cost. There will be a charge for paper copies per Washington State regulations.
